- In November 2023, off the coast of Japan, three CV-22 Ospreys were preparing for what should have been a standard training operation. Following a scheduled refueling and maintenance stop in Iwakuni, two of the aircraft departed first, tasked with conducting in-flight refueling with a C-130 and executing a free-fall water jump. A third Osprey was set to follow an hour later in a support role. Gundam 21 and Gundam 22 carried a total of 23 personnel, with 15 aboard the former and 8 in the latter. As they flew over the ocean, the aircraft maintained formation, executing what should have been a routine mission. However, 40 minutes into the flight, a mechanical issue surfaced on Gundam 22. The crew reported multiple chip burn advisories, an indication of potential engine trouble, yet chose to proceed with the mission.
